Thesis Title

Muscle Force Production and Neuromuscular Fatigue Responses to Neuromuscular Electrical Stimulation and Tendon Vibration

Citation

Neuromuscular electrical stimulation is an intervention that produces artificial muscle contractions and is used to rehabilitate people with motor impairments. However, a typical problem is the occurrence of rapid fatigue, which limits the training stimulus. This thesis investigated whether the addition of tendon vibration and concurrent activation of upper limb muscles could minimise fatigue and maximise torque produced by the calf muscles during neuromuscular electrical stimulation. The experiments in this thesis showed that superimposition of tendon vibration can increase the amount of torque produced by calf muscles, and that the addition of sustained handgrip contraction could further increase.

Citation

This research was a longitudinal mixed methods study that involved the delivery of an exercise intervention in brain cancer patients undergoing treatment. Three studies were conducted to examine post-surgical functioning, feasibility and safety, and a qualitative review of patients’ acceptability and experiences. This research highlighted functional deficits after surgery, limiting independence. The delivery of an exercise intervention during treatment is both feasible and safe. Clinically important improvements in aerobic functioning, lower-body strength and mobility were observed after cancer treatment. Participating in the exercise intervention provided patients with respite from their cancer journey and promoted positive coping ability towards future outlook.

Exploring the Acuity and Dependency of Adult Ward Patients Following Medical Emergency Team Review: The REMAIN Study

Citation

Hospitalised patients who deteriorate and require emergency medical review will most likely remain on a general ward after the review. Judgements about the level of care required to safely manage these patients on the ward after medical emergency team review are complex, but rarely incorporate a formal assessment of illness severity. This challenges staff and ward resource allocation practices and patient safety. This study measured the illness severity, in terms of physiological acuity and interventional dependency, of patients after emergency medical review. Novel statistical methods were used to identify then profile patient risk for death and potentially preventable adverse outcomes.

Effects of Ageing and Resistance Training on Intrinsic Motor Neurone Excitability

Citation

This thesis examined the age-related impairments in the function of motor neurones, the connection between our spinal cord and the muscles, and the use of exercise as a therapy to counteract this dysfunction. Older adults have reduced function of the motor neurones, which could contribute to muscle weakness and physical function impairments observed in this population. Exercise can be used as medicine to restore motor neurone function, contributing to improvements in muscular strength and general physical function in older adults.

Nursing Discernment and Response to Suffering at the End of Life in Acute Care: A Grounded Theory Study

Citation

This grounded theory study explored patient suffering at the end of life in acute care, as experienced by those caring for them. Being present at this time can challenge nurses, who face the multifaceted nature of non-physical suffering and a ‘fracture of personhood’. Nursing discernment and response to suffering were contextualised and interpreted. This generated new theoretical understanding not only of suffering, but of how the treatment-orientated paradigm of acute care impacts nursing’s identity and voice. Further analysis resulted in the theory of dying denied. Nursing identity and dying denied are interrelated, and a model of reciprocal compassion is proposed.

The Effects of an Educational Intervention on Exercise Advice Behaviours of Thai Oncology Nurses

Citation

This thesis evaluated the impact of an educational intervention based on the Theory of Planned Behavior that was designed to encourage oncology nurses in Thailand to provide exercise advice to cancer survivors. Following the intervention, a significant improvement was identified in the extent to which oncology nurses engaged in exercise advice behaviours. The study adds to knowledge about the importance of addressing attitudes and perceived behavioural control in relation to exercise advice behaviours when designing educational interventions to improve health promotion activities for cancer survivorship.
