Parchment (replacements)
'When you graduate from your course, you will be given a parchment. This is a document that confirms your completion of the course and conferral of your degree. It is different from an academic record because it does not list individual units.
You will receive your parchment upon the conferral of your degree. Your parchment cannot be issued before your graduation ceremony/conferral date.
My original parchment has been lost or damaged
If you have lost your original parchment, we can replace it. You will need to complete the statutory declaration at the end of the request for replacement parchment (RP form).
If your original parchment was damaged, please return it with your application for replacement.
Applying for a replacement
- Paying by credit card: complete the online request for a replacement parchment form to pay by credit card and mail your PDF receipt and all supporting documentation to the address on the top of the form.
- Paying by cash, cheque, or money order: complete the hard copy of request for replacement parchment (RP form) and mail the form and all supporting documentation to the address on the top of the form.
Applying from outside Australia
If applying from outside Australia where a statutory declaration is not available, complete an equivalent declaration attesting to the truth of your statement and witnessed by an officer authorised to witness an oath.
Alternatively make a declaration on a legal firm's letterhead attesting to the truth of your statement and have it witnessed by a solicitor registered in the country from which you are applying.
I have changed my name
A replacement parchment can be issued if you have changed your name. You will need to return your original parchment and provide certified copies of either your marriage certificate, birth certificate, or deed poll.
My original parchment was issued by another institution
If your lost parchment was issued by one of QUT's predecessor institutions (such as QIT, BCAE) you will be issued with a substitute parchment with an appropriate endorsement identifying the original institution.
Cost
$60 per replacement parchment which can be paid via QUTPay, at the cashier at the Student Centre enquiry counter on your campus, or by sending a money order or cheque payable to QUT with your application.

How long will it take
Please allow 10 working days for your replacement parchment to be processed, and avoid delays by submitting the required documents and payment with your form. Your replacement parchment will be sent by Registered Post (within Australia) or Air Mail (overseas) unless you pay an additional $30 to have it couriered.
Please note that during peak graduation periods (December and July) the issuing of replacement parchments could be delayed.
